Thank you for your interest in the Florida Health Survey!

We want to learn more about the health of Floridians. The Florida Department of Health is developing education programs that will improve the health of Floridians. To inform the education programs, the Florida Department of Health is conducting a survey in partnership with RTI International, an independent nonprofit research organization.

You can help by completing an online survey. The survey is short and will take about 10-15 minutes. The adult (age 18 or older) in your home with the next birthday should complete the survey. To get started, return to the home page, select the language you wish to use to complete the survey, and enter your 6-letter access code.

Here are a few important details about the survey:

• Participation is voluntary. You do not have to complete the survey if you don’t want to.
• Your answers will be kept private. We will never share answers or your address.
• We will never report just your answers. We will look at answers from all surveys together.
